5/31/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C arrived to the home of Ben-Nita to conduct individual session. QMHP-C informed Ben-Nita that this session would continue to focus on the previous session anger management to determine how she was feeling from the last session. Mother reports Ben-Nita continues to have issues with respecting those who are in authority. 2. Behavior: Ben-Nita presented hyperactive behaviors evident by her laughing at her mother when her mother was discussing with QMHP-C on how she is about to be 18 years old and she continues to engage in the behaviors she engages in. 1.Intervention: QMHP-C began session by inquiring about Ben-Nita’s emotional wellbeing. QMHP-C began session by reviewing from the last session. QMHP-C recapped from the last session. QMHP-C explored Ben-Nita's disrespectful behaviors in the home with her mother. QMHP-C discussed how Ben-Nita's disrespectful behaviors have continues to be displayed in the school setting (virtually as she continues to struggle with attending school on time or not attending at all) and with other authority figures. 2. Intervention: QMHP-C then engage Ben-Nita to express her feelings and thoughts to her mother concerning her behaviors and triggers that cause her disrespectful demeanor. QMHP-C actively listened to Ben-Nita and mother report incidents in the home that's caused some negative interaction among each other. QMHP-C inquired about Ben-Nita's expectations and how she would want to see things be different between her and mother. QMHP-C discussed age-appropriate consequences for Ben-Nita's actions and processed on how her behaviors are projected onto others. 1. Response: Ben-Nita was receptive to QMHP-C's suggestions for today's session. Ms. Shabazz mother reported that Ben-Nita has been talking back and saying things under her breath in the past days. She shared that Ben-Nita has been getting beyond herself lately. Ms. Shabazz reported that she didn't want to result other punishments for Ben-Nita. 2. Response: Ben-Nita denied everything reported by the mother which she failed to take responsibility for her actions. Ben-Nita became upset as she hate being confronted about her actions. QMHP-C had to redirect Ben-Nita as she walked away to calm down and returned to session. Ben-Nita reported that she can't talk to her mother when she's upset.
